My goal in the coming years is to focus on direct, personalized medicine. In addition to our current model, I'm excited to announce that my practice will be offering an Enhanced Wellness Care model at APG. As you may know, I am a second-generation healthcare provider and have been serving the needs of patients for over 30 years.

With the new Enhanced Wellness Care model, I will be able to provide my patients with the highest level of service. I will spend much more time with them, and discuss wellness plans, lifestyle management, prevention, and chronic disease management.

Although the APG Enhanced Wellness Care program personalized healthcare is sometimes referred to as concierge medicine, it's not. Our model goes far beyond "fee-for-access" practices. Patients participating in the Enhanced Wellness Care program pay a reasonable fee for a defined set of medically-relevant services focused on early detection, prevention and wellness, and services not traditionally covered by insurance.

Enhanced Wellness patients will see me personally at every visit. To ensure adequate time and attention for my patients, I will see only a limited number of patients in my Enhanced Wellness Care practice.

The Enhanced Wellness Care addition in my practice will be effective July 1st, 2024. 

With the Enhanced Wellness Care practice, patients will receive numerous services, such as:

  • 24-hour, seven-day-a-week availability with my personal cell phone number/email

  • Longer appointments with me every time with minimal-to-no waiting

  • Contact numbers for my dedicated office staff  

  • Same-day, next-day and Tele-health appointments, Monday-Friday. 

  • In-suite lab draws  

  • Expedited paperwork and prescription management  

  • Coordinating hospital care and specialist appointments 

I am pleased to offer you the opportunity to join my Enhanced Wellness Care practice before opening it to non-established patients. There are a limited number of spots available, so I encourage you to join as soon as possible if this is something you feel would benefit you.

The annual fee for APG Enhanced Wellness Care is $2,100. The membership fee is not covered by insurance and is an out-of-pocket cost for patients. This membership fee can be paid annually, quarterly or monthly. The membership fee includes additional amenities identified above not covered by your insurance.

The membership fee does not cover professional clinical services covered and billed through Medicare or other health insurance plans. In addition to participating in Medicare, I will remain an in-network provider for all insurance plans that APG accepts. 

I consider it both an honor and a responsibility to care for those who have entrusted their health to me over the years. By making this addition, I will be able to extend my career as a physician and continue to serve my patients.

FAQ

  • Officially July 1st is the start date for Enhanced Care appointment availability.

  • Yes, we have annual, quarterly and monthly options. There are a limited number of spots in the Enhanced Wellness Care practice model and this secures your place.

  • If signing up between now and July 1, 2024, your plan will renew on July 1st each year going forward. Annual payments will be renewed- July 1, 2025. Payment plans will be based on sign-up date and depending on their plan, charges will end after their 4th or 12th payment and then start again on July 1st.

    If signing up after July 1, 2024, your plan will renew on the first day of the month after 1 year.

  • You can sign up here on the web site by selecting one of the payment options listed here. Payments can be made as a one-time payment, quarterly payments or monthly payments. You can also sign up by calling the enhanced care phone number (618-520-0821) and submit payment over the phone with our Enhanced Wellness Care manager. You may also pay in person at the office with a check.

  • Yes, APG requires 30 days notice. If paid in full or quarterly, a pro-rated amount will be returned.

  • When you sign up for our Enhanced Wellness Care Program, you’ll receive access to Dr. Green’s personal cell phone and his personal email. Dr. Green will be accessible to you 24/7.

  • This is not insurance, so we do not file with your insurance company for the Enhanced Wellness Care program fee. We do, however, file with your insurance for routine services ordered and followups.

  • In most cases, the Enhanced Wellness Program fee is not covered by FSA/HSA. Check with your plan administrator to confirm.

  • No. Dr. Green will see patients in the same office at 4414 West Center Drive, Alton, IL. There is a different phone number for Enhanced Wellness Care patients, 618-520-0821.
